What are the Possibilities of Antibiotics in Acute Pneumonia

Introduction Modern ideas about the nature of acute pneumonia (АР) are the main obstacle to its successful treatment. On the one hand, AP is not considered a contagious infectious disease, does not require isolation of patients and the observance of special precautions, and the known and widespread today pathogens of AP belong to representatives of symbiotic microflora of healthy people. On the other hand, all hopes for the successful treatment of the disease are pinned only on antimicrobial effects. “Antibiotics alone” is the initial treatment of all patients with AP. The emergence of more and more antibiotic resistant strains of microorganisms is a natural consequence of long-term and intensive use of such treatment. Adaptability of microorganisms as representatives of the biological world is their essential feature. The ongoing process of confrontation between the natural presence of microbes in many areas of our body and the means of its suppression is ...

Burden of Chronic Obstructive Pulmonary Disease in India: Status, Practices and Prevention

Abstract Chronic obstructive pulmonary disease (COPD) is affecting 251 million lives globally and it causes 3.15 million deaths per year More than 90% COPD-related deaths happen in low and middle-income countries. In India, three out of five leading causes of mortalities constitute non- communicable diseases whereas COPD is the second biggest cause of death. The prevalence of COPD has increased by 29.2% by 2016 which is a serious public health concern. The clinicians require treating COPD as per the evidence-based guidelines and managing the comorbidities. To avert the cost of care, an integrated and highly efficient system is required which can promote healthier lifestyle and self-management of the symptoms through a team-based approach comprising different healthcare professionals for delivering planned care. Moreover, smoking and other risk factors should be controlled considering the future benefits of the nation. Also, air pollution and other environmental r...

Covid-19 Era Post Viral Vagal Neuropathy Presenting as Persistent Shortness of Breath with Normal Pulmonary Imaging

Opinion All that is shortness of breath is not pulmonary. We have identified a cohort of 18 outpatients in a general otolaryngology practice in New York City who presented with persistent shortness of breath (SOB) despite normal pulmonary imaging. All patients had cough, fever and SOB between March 15 - May 15, 2020 during the height of the COVID-19 pandemic in New York City, with the SOB persisting for 4 - 12 weeks at the time of their presentation. 3/12 were positive for COVID-19 IgG antibodies, one of whom also tested positive with a nasal swab polymerase chain reaction (PCR) assay. Due to limited access, only 12 patients were able to obtain an antibody test and only 7 were able to get the PCR test. Of the 4 patients who had spirometry as part of their pulmonary investigation, the flow volume loop showed flattening of the inspiratory curve in all 4. History revealed that the SOB was generally not waking these patients up at night nor was it affecting their e...
